给Lead公司发 W-9，如何告诉他们我的Affiliate ID?

以下是一家Lead公司让我传真 W-9，其中的一段话：

Please type or print clearly and initial the lower right corner of each
page and include your Affiliate ID number in the upper right corner of
each page.

说的什么意思阿？

让我把 Affiliate ID写在哪儿？

另外，有些公司没说要在W-9里面写自己的Affiliate ID，我把 W9发过去，公司能知道这是谁的W9么？

看后半句 Affiliate ID number 是让写在每页的右上角，

但前半句 initial the lower right corner of eachpage， 是说缩写签署在每页右下角，也是指的Affiliate ID number 的吗？
