W-9 tax form 是什么东西啊

注册了一家网站，来了一封信，说是批准申请之前需要回答他的一些问题，还要传真W9表格。
请问这些问题怎么回答啊？还有W9怎么填呢？

1.How will you get people to view these promotions ie get visitors to your site etc.?
2. How many leads do you expect to generate daily or monthly?
3. Do you offer an incentive program or sweepstakes? If so, please explain your program.
4. Please complete the W-9 tax form located on the following webpage and fax it back

2.As we only provide financial services to UK（我用美国站申请滴）, how will you ensure that we only receive traffic from UK.

第一贴知道什么意思，但是怎么回答合适呢？比如每天能产生多少LEAD。
